　腰部根性疼痛の原因として，腰椎椎間板ヘルニアや腰部脊柱管狭窄症がよくみられる疾患である．その他にも第5腰椎(以下，L5)椎間孔外側障害があり，見逃されやすい疾患として報告されている．L5椎間孔外側障害を生じるものとして，L5/仙椎間外側型ヘルニア(以下，L5/S L-LDH)やFar-out症候群(以下，FOS)などが挙げられる．今回，L5/S L-LDHを対照群として比較することにより，変性側弯によるFOSの特徴についてX線画像〔L5横突起-仙骨翼間の距離(T-A距離)，およびL4傾斜角，L5傾斜角，L1-S1 Cobb角〕を中心に検討した．FOSの最大の特徴としてはT-A距離3mm以下だけでなく，症状側にL5椎体が傾き，またそれだけでなくL1椎体は代償性に反対側に傾く傾向であった．

　Lumbar disc herniation and lumbar spinal canal stenosis are common causes of lumbar radicular pain, and extraforaminal entrapment of the fifth lumbar spinal nerve is an easily overlooked cause.　Lateral disc hernia at L5/S (L5/S L-LDH) and far-out syndrome (FOS) were contained for the extraforaminal entrapment of the fifth lumbar spinal nerve.　We investigated the radiographic characteristics of FOS 〔distance between the L5 transverse process and ala of the sacrum (T-A distance〕, L5, L4 tilt angle, and L1-S1 Cobb angle by comparing the values in an L5/S L-LDH group as a control group.　The L5 vertebra was tilted toward the symptomatic side and the L1 vertebra was tilted to the other side.
