今回われわれはPasteurella haemolyticaが原因と考えられた頚椎化膿性脊椎炎の1例を経験したので報告する.患者は52歳の男性で,頚部痛と右上肢痛を主訴に受診し,単純X線とMRIから化膿性脊椎炎が疑われ入院となった.椎間板針生検を施行し,Pasteurella haemolyticaが検出された.抗生剤の投与および安静臥床治療で症状は軽快した.本症例ではペットである犬との過度の接触が感染経路として考えられた.
We report a case of cervical spondylitis caused by Pasteurella haemolytica. A 52-year-old man with neck and upper right extremity pain was found in plain radiographs to have osteolytic change in the C5 and C6 vertebral bodies. MR showed osteolytic change at C5 and C6. The histological diagnosis was made by examining specimens obtained by needle biopsy, which detected Pasteurella haemolytica. The patient was treated conservatively with antibiotics for two months and symptoms were relieved. We suspected his dearly kissing of dogs contaminated him with Pasteurella haemolytica.
