　関節リウマチの治療中の67歳の女性が，環軸椎固定術後10年で軸椎下後弯変形および第3頚椎前方亜脱臼を来した．C3/4レベルでの著明な脊髄圧迫による四肢麻痺を認めた．環軸椎固定術とC5-6椎体間の自然癒合によりC3/4にストレスが集中し，C3前方亜脱臼が生じたと考えられた．軸椎下後弯変形とC3前方亜脱臼に対し前方矯正固定術を行い，神経症状の改善を得ることができた．また，ハローベストでC3/4のわずかな可動性を確認することができ，術式決定の際に有用であった．

　A 67-year old woman with rheumatoid arthritis exhibited a subaxial kyphotic deformity and C3 anterior subluxation ten years after undergoing C1-2 posterior wiring for the treatment of atlantoaxial subluxation. The atlantoaxial fusion and the spontaneous fusion of C5-6 were thought to have led to the subaxial kyphotic deformity and C3 anterior subluxation seen in this case. The patient suffered from tetraplegia. The subaxial kyphotic deformity was corrected by anterior surgery, and the patient successfully recovered from her neurological deficit. The use of a Halo brace assisted in the selection of a surgical method.
