Two cases with the EEG pattern of 6, 7 & 14 c/s positive spikes were examined post-mortem to clarify the pathophysiology of the generation of positive spikes.
In the first case, 6 & 7 cis positive spikes were noted at the transitional stage of diabetes insipidus to panhypopituitarism. An ectopic pinealoma of the hypothalamus was found post-mortem.
In, the second case, 6, 7 & 14 c/s positive spikes were observed at the same time that dysphagia, dys p-nea and spastic quadriparesis were present. A central pontine myelinolysis was found in this case.
No lesions were found in the cerebral cortex and white matter which were considered a possible cause of the positive spikes.
These two cases suggest that positive spiking may be a characteristic EEG evidence of brain stem dys-function.
