抄録

　脳血管障害後に妄想が出現することは従来から報告されている。しかし，その関連が詳細に検討されたことは少ない。我々は，右側頭・頭頂葉出血後に，短期間で嫉妬妄想が体系化した1症例を報告した。本症例の嫉妬妄想の形成にはさまざまな要因が考えられたが，脳出血にて生じた相貌失認，視空間障害，視覚性記憶障害などの高次脳機能障害が妄想形成の一つの要因になった可能性を指摘した。また，脳血管障害の中でも特に右半球損傷が妄想形成の背景となり得る可能性を指摘した。

　It has been reported that delusion appears after cerebro-vascular diseases. However, the association between the two has rarely been studied. We report one case in which a delusion of jealousy was systematized shortly after right temporo-parietal hemorrhage. Various factors may be involved in the formation of delusion of jealousy in this case, but we have pointed out the possibility that cognitive dysfunctions such as prosopagnosia, visuospatial disorder and visual memory disorder arising from cerebral hemorrhage have constituted one of the factors for formation of this delusion.

　We have also pointed out the possibility that especially injuries in the right hemisphere among the cerebro-vascular diseases can be the background of the formation of this delusion.
