要旨●教科書にCrohn病の肛門病変の病理組織学的特徴の記載はあるが，病理組織像を提示した解説はみられないため，その特徴は十分理解されていない．腹痛など腸管病変としての症状が出現する前の，より早期の段階でCrohn病と診断することは患者の良好な予後に寄与すると考えられる．Crohn病の肛門病変においても病理組織学的特徴は基本的には腸管と同様であるが，膿瘍などの二次的変化の影響を加味して組織像を評価することが鑑別において重要である．

　The histological features of anal lesions in patients with Crohn's disease remains unclear, possibly because it is described in textbooks without the histological pictures. An early diagnosis of Crohn's disease with asymptomatic intestinal lesions promotes good prognosis of the patients. In addition, the histological characteristics of anal lesions in Crohn's disease are fundamentally the same as those of intestinal lesions. However, secondary development, including abscess formation, should be considered while evaluating the histological features of the lesions.
