要旨　早期胃癌の深達度診断を，X線，内視鏡，超音波内視鏡（EUS）とで比較検討した．32症例の早期癌において，正診率はX線で62％，内視鏡で63％，EUSで69％であった．32症例中16例が陥凹型早期癌で，この型における正診率は，X線，内視鏡ともに53％，EUSでは71％であった．陥凹型早期癌におけるEUSの誤診の唯一の理由は，超音波画像上で粘膜下層を中断させた，併存潰瘍による線維化に起因した．retrospectiveにみると，エコー上での中断した粘膜下層先端の形を検討することにより，線維化の存在はEUSで診断可能であることが明らかとされた．すなわち，中断した粘膜下層の先端における，粘膜側への“引きつけ”，“先細り”“凸状”などの形は，線維化を伴った症例に特徴的であった．更に，エコー上での筋層は，粘膜下層に線維化を持つ例では変化なく，筋層にまで到達した線維化例では，筋層の低エコー腫脹，粘膜側への引き上げなどの変化がみられた．線維化合併例におけるm癌とsm癌とのエコー上での鑑別は，腫瘍を疑わせるmass lesionが病変部の辺縁にみられるかどうかにあると思われた．

　X-ray, endoscopy and EUS were compared with respect to the accuracy rate in diagnosing depth invasion of early gastric cancer. In thirty-two cases of early cancer, the accuracy rates were 67% by x-ray, 63% by endoscopy and 69% by EUS. However, when limited to the depressed type of early cancer (16 of 32 cases), the accuracy rate obviously differs among these diagnostic modalities, i.e., 53% by either x-ray or endoscopy, and 71% by EUS.

　The only cause leading to the miss-diagnosis by EUS in the depressed type early cancer was the presence of fibrosis due to the concurrent ulcer which interrupted the submucosa on echograms. Retrospectively, it became clear that the existence of fibrosis could be diagnosed by EUS based on the shapes of the tip of the interrupted submucosa on echograms. Namely, “fusion” toward the mucosal side, “tapering” and “convex” in the tip of the interrupted submucosa were the findings characteristic to the fibrosis due to the concurrent ulcer.

　Furthermore, the muscularis propria on echograms was intact in cases with submucosal fibrosis. When the fibrosis reached to the muscularis propria, low echoic swelling or tugging toward the submucosa was seen in the muscularis propria.

　The echographical differentiation between the mucosal and the submucosal cancer in cases with fibrosis was thought to be possible by determining the presence or absence of mass suggesting tumor, at the margin of the lesion.
