　胃切除後の残胃に発生する癌は，癌の発生母地や発生機序に関連して興味があるが，残胃の癌を論ずる際には，残胃に見出された癌が最初の手術における胃癌の再発であったり，切除の時点で既に残胃に存在していた可能性をできるだけ除外する必要がある．このため，残胃の癌の概念や定義に関しては種々の見解が主張されている．すなわち，最初の胃切除の原因となった疾患が悪性腫瘍である場合と良性疾患である場合を区別し，更に，最初の手術後，残胃に癌が発見されるまでの期間を5年，10年，15年以上などと規定する考え方である．

　われわれは18歳で十二指腸潰瘍として2/3胃切除術を受け，19年後に残胃吻合部に，いわゆるgastritis cystica polyposa（Littler），stomal polypoid hypertrophic gastritis（遠城寺）の所見と共存して，Ⅱa型早期胃癌が発生していた症例を経験したので報告する．

　A 18-years-old man had undergone subtotal gastrectomy and gastrojejunostomy (Billroth Ⅱ) because of duodenal ulcer. After 19 years in the age of 37 a polypoid lesion is found in anterior wall of the gastric remnant adjacent to the gastrojeiunal anastomosis and clinically diagnostized as type Ⅰ early gastric carcinoma. Histological examination of total resected stomach remnant reveals well differentiated tubular adenocarcinoma, type Ⅱa early gastric carcinoma (m), probably developed from the lesion of gastritis cystica polyposa (Littler) or stomal polypoid hypertrophic gastritis (Enjoji).
