上部消化管出血の治療方針の決定に関する内視鏡検査の意義―内視鏡的経過観察による厳重な監視体制の確立と手術適応基準の設定について

Evaluation of Emergency Endoscopy on Surgical Treatment of Upper GI Bleeding

　上部消化管出血に対する緊急および早期内視鏡検査の目的として，われわれは，出血巣の確認，出血持続の判定，内視鏡的止血術の施行，緊急手術の適応決定を指摘してきた．このうち，診断面における内視鏡検査の有用性については，われわれもこれまでに多くの報告を試みてきた^{1)2)15)～18)}．

　ところで近年，われわれは治療面での本検査の有用性を検討する意味で，手術適応の基準を内科側の視点に立って作成し，外科医の協力の下に，臨床の場に適用してきた．今回，この成績をもとに，上部消化管出血の外科的治療の適応について若干の考察を行ったので報告する．

　Upper gastrointestinal bleeding must be treated on the basis of the patient's general condition evaluated from the severity of bleeding and the estimation of a change of the general condition judged from repeated endoscopic observations of the bleeding site. Thus, it can be avoided to miss the opportunity for surgical operation or to operate on in vain. Judging from this point of view, our criteria of the surgical operation for upper gastrointestinal bleeding proved to be sufficient in their clinical application.

　We were able to make much of the usefulness and safety of the emergency endoscopy for the treatment of upper gastrointestinal bleeding.
