要旨　患者は77歳，男性．1993年7月ごろから腰痛のため，近医から鎮痛剤の投与を受けていた．8月17日からタール便が出現し，8月21日当院を受診し入院となった．同日の内視鏡検査では，胃角前脚にいわゆる“そなえもち”様の形態を呈した巨大な隆起性病変を認めた．病変の基部は表面平滑で，頂部には小隆起の突出を認めた．第4病日には頂部の小隆起は更に突出し，第11病日には突出部は内腔に伸び出すように形態変化が認められた．突出部の生検では，肉芽組織のみであったため，同部の一部をポリペクトミーしたところ，平滑筋腫であった．その後も腫瘍からの出血が持続したため，9月25日幽門側胃亜全摘術を施行した．腫瘍は7.5×4.0×3.5cm大の弾性軟，表面分葉状を呈した平滑筋腫であった．短期間に形態変化を呈した胃平滑筋腫はまれであり，若干の文献的考察を加え報告した．

　A 77-year-old man was admitted to our hospital complaining of tarry stools. Emergency endoscopic examination revealed a huge elevation with a wide base on the anterior wall of the lower body of the stomach. This tumor looked like a “rice-cake offering”. Endoscopic examination on the fourth hospital day showed that the top of the tumor protruded. The shape of the tumor gradually changed day by day. We therefore performed polypectomy of the protrusion. Histological examination of the polypectomy specimen showed a leiomyoma. As bleeding from the tumor continued subsequently, open surgery was carried out. Histological study of the resected specimen revealed a leiomyoma. This was a rare case of leiomyoma that changed in shape over a short period.
