要旨　噴門部早期癌診断の実態を前期（1978～1986年）と後期（1987～1996年）に分けて検討した．検討対象は前期78病変（早期癌29病変），後期86病変（早期癌54病変）である．全早期癌に対する噴門部早期癌は前期4.6％，後期8.1％，噴門部癌における早期癌の比率は前期37％，後期63％，早期癌の中のm癌比率は前期41％，後期56％でいずれも診断能は向上していた．全噴門部癌の40％は41mm以上の病変で，この領域の初期病変の診断能は向上していなかった．

　To investigate the realities of diagnosis of the cardiac early cancer of the stomach, our cases were divided into two subgroups as follows: the first term (1978 to 1986); 78 lesions including 29 early cancers and the last term (1987～1996); 86 lesions including 54 early cancers. The proportions of early cardiac cancers to whole early cancers were 4.6% in the first term and 8.1% in the last term. The proportion of early cardiac cancers to whole stage of cardiac cancers were 37% in the first term and 63% in the last term, and those of mucosal cancers to early cardiac cancers were 41% in the first term and 59% in the last term. These results suggested improvement of diagnostic ability. However, forty percent of the whole cardiac cancers were larger than 40 mm in size, therefore the diagnostic ability to detect small lesions in the cardiac region did not seem to be improved.
