要旨　1996年12月から1998年4月までの1年5か月の期間に全大腸内視鏡検査（total colonofiberscopy；TCF）が施行されprospectiveに内視鏡的，病理組織学的に検討された潰瘍性大腸炎は70例であった．70例から全大腸炎型，区域性病変を除外した直腸炎型23例，左側大腸炎型24例の計47例を対象とし以下の成績を得た．①非連続性の虫垂開口部病変の頻度は47例中16例（34.0％）であった．②虫垂開口部を含む非連続性病変のうち虫垂開口部に限局したものは16例中10例（62.5％）で他の6例（37.5％）は盲腸，上行結腸までの比較的広範囲な病変であった．③虫垂開口部病変を有する群と有さない群との年齢，罹病期間，病型，重症度には有意な差はなかった．④虫垂開口部病変と肛門側病変の内視鏡的重症度は有意な相関を示した．以上の成績と症例を呈示し考察として病期によって虫垂開口部病変は消退するため，その頻度は更に高率であること，非連続性の右側大腸炎は虫垂開口部にとどまらず比較的広範囲なものが少なくないことを述べる．

　Forty-seven patients with distal (left side and proctitis) colitis were examined using total colonoscopy with regard to skipped lesions of the appendiceal orifice endoscopically and histologically. Sixteen patients (34.0％) of the 47 patients had skipped lesions around the appendiceal orifice. Comparing background features of patients with and without this appendiceal lesion, we could detect no difference in age, activities, duration of the disease, location of the lesions, clinical course, and severity. However, there was correlation between endoscopic grades of the lesions at the rectum and at the appendiceal orifice using Matts' classification. Six patients (37.5％) of 16 patients with appendiceal lesions showed an extended lesion from the appendiceal orifice to the ascending colon. Therefore, we suggest that appendiceal lesions in distal colitis are not infrequent and they probably extend to colitis on the right side.
