要約　目的：小眼球症に伴うuveal effusion syndrome（UES）に対して，強膜開窓術を施行し，治療しえた症例について報告する。

症例：83歳，男性で，前医での両眼白内障手術後の経過観察中に，左眼に遷延する黄斑浮腫を生じたため，前医でトリアムシノロンのテノン囊下注射を数回受けた。その後，漿液性網膜剝離（SRD）が黄斑にも及び，左視力は（0.02）まで低下した。弘前大学医学部附属病院眼科に紹介された際には，眼軸長が右15.91mm，左15.92mmであり，両眼の小眼球症と診断した。さらに左眼はUESを生じている状態と診断し，強膜開窓術を施行した。具体的には，鼻側上方および耳側上方には強膜半層切除を置き，鼻側下方および耳側下方には同様の強膜半層切除のうえで，強膜深層を切開し，小さな強膜切除も行った。術後SRDの改善が確認されたが，遷延する黄斑浮腫によると考えられる黄斑部の変性や浮腫残存により術後視力（0.03）にとどまった。術後1年8か月が経過し，左眼の黄斑浮腫はみられるが，SRDは再発していない。

結論：小眼球症のUESにSRDを生じた症例に対して強膜開窓術を実施し，SRDの治療は可能であった。より早期の手術によって視力予後の改善が期待できると考えられた。

Abstract　Purpose：We report a case of successful treatment of uveal effusion syndrome（UES） associated with microphthalmia using scleral fenestration surgery.

Case：An 83-year-old male developed persistent macular edema in his left eye during follow-up after bilateral cataract surgery at a nearby hospital. Consequently, he received several sub-Tenon injections of triamcinolone. Subsequently, serous retinal detachment（SRD） extended to the macula, causing his left visual acuity to decline to 20/1,000.

　At the time of presentation to our department, the axial lengths were 15.91 mm in the right eye and 15.92 mm in the left eye, leading to a diagnosis of microphthalmos in both eyes.

　The left eye was diagnosed with nanophthalmos and uveal effusion, and a left scleral window procedure was performed. In the inferior nasal and inferior temporal regions, a scleral deep layer incision was made over a similar scleral half-layer resection, and a small scleral resection was also performed to create a scleral window.

　At 1 year and 8 months postoperatively, macular edema persists in the left eye, but no recurrence of SRD has been observed.

Conclusion：Although SRD developed in UES of nanophthalmos and was treatable via scleral fenestration at two inferior sites, it was considered that earlier surgery should have been performed to better preserve visual acuity.
