要約　目的：両眼の水晶体亜脱臼を伴う臨床的Marfan症候群に併発した両眼の中心性漿液性脈絡網膜症を経験したので報告する。

症例：41歳，男性。父親は高身長，水晶体偏位の既往をもつ。両眼水晶体偏位の所見から臨床的にMarfan症候群の診断となった。左眼の見えにくさを自覚し近医を受診し，両眼の水晶体偏位，左漿液性網膜剝離の所見がみられたため，精査・加療目的で帝京大学医学部附属病院眼科に紹介され受診となった。

所見：視力は右0.3（0.4×−0.50D（）cyl−1.50D 180°），左0.3（n.c.），眼軸長は右22.54mm，左22.04mmとやや短眼軸であった。両眼水晶体の上方への水晶体偏位がみられた。左黄斑部に漿液性網膜剝離がみられた。経過のなかで両眼中心性漿液性網膜剝離の再発，自然消退がみられた。

結論：Marfan症候群は結合組織に発現するfibrillin-1をコードするFBN-1遺伝子の変異により眼軸長延長と強度近視を伴うことが多い。今回の症例は比較的短眼軸長であり，非典型的ではあるがMarfan症候群に中心性漿液性脈絡網膜症が併発する可能性があることが示された。今後同様の症例の報告と検討により，両疾患の関連性が明らかになる可能性がある。

Abstract　Purpose：We reports a case because we experienced central serous chorioretinopathy concomitant with clinical Marfan syndrome and lens subluxation in both eyes.

Cases：A 41-year-old man was clinically diagnosed with Marfan syndrome due to his father's height and lens deviation in both eyes. Realizing that his left eye was difficult to see, he visited a nearby doctor and was referred for detailed examination and treatment because he had findings of lens deviation of both eyes and serous retinal detachment in his eyes.

Findings：His visual acuity was right 0.3（0.4×−0.50D（）cyl−1.50D 180°）, left 0.3（n.c.）, and the axial length was 22.54 mm on the right and 22.04 mm on the left. Lens shift in the upper direction was observed in both eyes. Serous retinal detachment was found in the left macula. During the course of treatment, recurrence and spontaneous regression of binocular central serous retinal detachment were observed.

Conclusion：Marfan syndrome is often accompanied by axial length prolongation and severe myopia due to a mutation in the connective tissue gene fibrillin-1（FBN-1）. In this case, the axial length was relatively short, which is atypical, but it suggests that central serous chorioretinopathy can occur concurrently with Marfan syndrome. Future reports and examinations of similar cases may clarify the relationship between these two conditions.
