要約　目的：副鼻腔真菌症は，しばしば眼窩内に波及し視機能障害や生命予後の悪化を招くため，早期の診断と，投薬および外科的デブリードマンによる治療介入が求められる。従来，内視鏡による副鼻腔アプローチが主とされ，前方到達法による眼窩手術の報告はきわめて少ない。今回，前方到達法手術により，視機能および全身状態の良好な経過を示した1例を経験したので報告する。

症例：78歳，男性。1か月前より左眼奥の痛みとしびれを自覚した。前医で左眼球突出，眼瞼腫脹，外転障害を認めMRIを施行したところ，左上顎洞から眼窩内へ進展する腫瘤性病変を認めた。耳鼻咽喉科にて緊急で上顎洞開放術と生検が行われたが診断に至らず，当院紹介となった。

所見：当院受診後，経過から副鼻腔真菌症を疑い，外科的手術を計画した。病変の主体が眼窩内であったため，下眼瞼睫毛下切開による前方到達法と，鼻内視鏡による上顎洞アプローチを併用した。術中迅速病理では糸状菌の集簇と炎症細胞浸潤を確認した。術後，ボリコナゾールの投与を開始し，視機能および全身状態が改善した。

結論：眼窩へ進展した真菌性副鼻腔炎に対し，前方到達法を併用した外科的デブリードマンにより，視機能と全身状態の改善が得られた。病変の局在や眼症状に応じ，前方到達法の選択が有効である。

Abstract　Purpose：Paranasal sinus fungal infections often extend into the orbit, leading to visual dysfunction and poor prognosis. Early diagnosis and treatment with antifungal medication and surgical debridement are essential. While endoscopic sinus surgery is the standard approach, reports of orbital surgery via an anterior approach remain rare. We report a case in which the anterior approach resulted in favorable visual and systemic outcome.

Case：A 78-year-old man presented with left orbital pain, numbness, proptosis, eyelid swelling, and abduction limitation. MRI revealed a mass extending from the left maxillary sinus into the orbit. Although emergency sinus surgery and biopsy were performed at another hospital, the diagnosis remained unclear, and he was referred to our department.

Findings：Based on the clinical course, fungal sinusitis was suspected, and surgical debridement was planned. As the main lesion was located in the orbit, we performed an anterior approach through a subciliary incision combined with an endoscopic maxillary sinus approach. Intraoperative frozen sections revealed filamentous fungal aggregates with inflammatory infiltration. Postoperatively, voriconazole was administered, and both visual function and systemic condition improved.

Conclusion：In cases of fungal sinusitis extending into the orbit, surgical debridement with an anterior approach can be effective for improving visual and systemic outcomes. The anterior approach should be considered depending on lesion localization and ocular findings.
