Abstract

 The elucidation of the molecular pathophysiology of Alzheimer’s disease(AD)enabled the clinical implementation of the disease-modifying therapies(DMTs)of AD that were developed based on the precise diagnostic and evaluation methods for the evaluation of trial drugs, and clinical observational studies utilizing the imaging and fluid biomarkers. The application of DMTs to the earliest, asymptomatic stage of AD is paving the way toward the secondary prevention of AD on high-risk elderly individuals. In this article, the author provides an overview on the advances in our molecular mechanistic research on the pathogenic molecules of AD, e.g., amyloid β and tau, and then the outcomes of the Japanese AD Neuroimaging Initiative(J-ADNI)clinical study, which was conducted aiming at developing and standardizing the diagnostic methods, construction of database and infrastructures for the global clinical trials of AD DMTs in Japan. I will describe the ongoing clinical research activities on the establishment of trial-ready cohort for the prevention trials of the asymptomatic, preclinical stage of AD, as well as the prior experiences on global clinical trials for AD DMTs, as well as the recent breakthrough toward the disease modification of AD. I would like to provide a comprehensive consideration on the current perspectives for the very early diagnosis, treatment and prevention of AD.
