生検時の臨床像と病理組織像が乖離して診断に難渋したspindle cell melanoma

A challenging case of spindle cell melanoma due to discrepancy between the clinical presentation and the histopathological findings of biopsy specimen

・spindle cell melanoma（SCM）は悪性黒色腫のうち1～14％を占め，組織学的診断のむずかしさからほかの上皮系腫瘍，軟部腫瘍と誤診されやすい．

・SCMは臨床的に黒色調を呈さず，病理組織学的に腫瘍細胞のメラニン産生も乏しく，免疫組織化学ではMelan AやHMB45が陰性となることが多いため，一般的な悪性黒色腫と異なる点が多く，注意を要する．

・SOX10はメラノサイト系病変で陽性率が高く，SCMの診断に有用である．
