変形性関節症の病理と病態

Bone and Cartilage Diseases and Regeneration. Destruction of the articular cartilage in osteoarthritis.

　変形性関節症（osteoarthritis：OA）は，整形外科領域で最も頻度の高い関節疾患である。OA関節軟骨の変性・破壊は，細胞外マトリックスの分解・消失，細胞外マトリックスの産生低下，軟骨細胞死などに起因する。細胞外マトリックスの分解には，MMP（matrix metalloproteinase）遺伝子ファミリーやADAMTS（a disintegrin and metalloproteinase with thrombospondin motifs）遺伝子ファミリーのメンバーが中心的な役割を果たしている。 　本稿では，OA関節の病理と病態について述べ，上記メタロプロテアーゼの関節軟骨破壊とリモデリングにおける役割について解説する。

　Osteoarthritis（OA）is the most common joint disease characterized by destruction of articular cartilage induced primarily by mechanical stresses to joints. Destruction of the articular cartilage in OA is caused by combination of increased degradation of ECM（extracellular matrix），decreased production of ECM and chondrocyte death. Members of the MMP（matrix metalloproteinase）and ADAMTS（a disintegrin and metalloproteinase with thrombospondin motifs）gene families are considered to play a key role in the degradation of cartilage.
