全身麻酔後に対麻痺を来し，転換性障害が疑われた1症例

Paraplegia after General Anesthesia in a Patient Who was Suspected of Having Conversion Disorder

要旨

　全身状態良好な若年患者が全身麻酔後に対麻痺を来し，転換性障害が疑われた症例を経験した。脳神経内科の診察で自発的な動きと神経診察での徒手筋力が一致せず，本症が疑われた。症状はその後改善した。周術期に神経学的に説明のつかない神経症状を来した場合は本症を考慮しつつ検査を行い，早期に対応することが重要と思われた。

　We report the case of a young, otherwise healthy patient who developed paraplegia following the use of general anesthesia in an endoscopic sinus surgery conducted for chronic rhinosinusitis, and was subsequently suspected of having conversion disorder（sometimes called ‘functional neurological symptom disorder）.’ He demonstrated a discrepancy between his self-perceived muscle strength and the findings of the neurological examination. After the exclusion of organic etiology, his symptoms subsequently resolved spontaneously.

　In cases in which unexplained neurological symptoms arise during the patient’s perioperative period, it is essential to consider conversion disorder, conduct a comprehensive evaluation, and initiate timely management.
