要旨
腰椎後方椎体間固定術（L4-5）の術直後に全身強直性痙攣が出現した。術中に低酸素血症はなかったが，コンピュータ断層撮影（CT）画像では低酸素脳症を疑う所見であった。今回の病態は硬膜外ドレーン陰圧吸引による急激な脳脊髄圧低下で生じたpseudohypoxic brain swelling（PHBS）と考えられた。
A 75-year-old man exhibited systemic tonic convulsions immediately after the posterior inter-body fusion for the lumbar vertebrae L4-5. Despite observing obvious hypoxemia during the operation, CT image showed changes unique to hypoxic encephalopathy due to pseudohypoxic brain swelling（PHBS）. Anesthesiologists should pay attention to PHBS in a case of convulsions after lumbar surgery.
