 Safe respiratory monitoring for sedation without intubation is discussed. Inspection and auscultation are essential for early detection of apnea and hypoventilation, which may lead to serious accidents;however, the usefulness of continuous monitoring of the respiratory rate in addition to inspection and auscultation is not well known. All respiratory monitoring devices have their own advantages and disadvantages, and the use of two or more monitoring devices that can overcome each other’s disadvantages would make the diagnosis of apnea and hypoventilation easier. This article shows that the monitoring of oxygenation with a pulse oximeter and monitoring of the respiratory rate by 2 methods, i. e., capnometry and acoustic respiratory monitoring, would make it easier to differentiate between mouth breathing and apnea. Future development of more accurate respiratory monitors is awaited.
