Background: Nipple-sparing mastectomy (NSM) is increasingly preferred for breast cancer, and prophylactic mastectomies are expected to rise with genetic testing. This trend makes immediate one-stage breast reconstruction desirable. For safe reconstruction, complete muscular coverage of the silicone breast implant (SBI) is essential. We employ a technique in which the SBI is entirely covered by the pectoralis major muscle (PMM) and the partial latissimus dorsi (partial LD) muscle in patients with large breasts.

Methods: When breast volume is (i) less than about 250 mL, the SBI is covered with the PMM alone or with the serratus anterior; (ii) when over 250 mL, the PMM and ventral half of the LD (partial LD) muscle cover the caudal and lateral SBI.

Results: This technique was applied in five patients. In three, the reconstructed breast closely resembled the original. One patient developed skin necrosis involving the nipple-areola complex; however, as the SBI was completely covered with muscle, healing was achieved through debridement and skin grafting, preserving the implant. Another patient developed postoperative capsular contracture following insertion of a slightly smaller SBI.

Conclusion: This method enables near-original breast reconstruction in one stage and offers advantages for postoperative management and safety.
