 Following the 2019 recall of Allergan Natrelle 410, the breast reconstruction implant market underwent significant changes. Although insurance-covered implants with a lower risk of BIA-ALCL were introduced, the number of implant-based reconstructions in Japan only recovered to 4,397 cases in 2022, maintaining its position as the most common method. By late 2022, three manufacturers—Abbvie, Gunze Medical, and Establishment Labs—were available in Japan, providing surgeons with more choices but also increasing the complexity of implant selection.

 This study investigates how different breast implants change shape in response to body positioning. Using a simulated system, we measured the projection and Maximum Point of Projection (MPP) of 11 implant types from three manufacturers when transitioning from a horizontal to an vertical position. Results showed that anatomical implants maintained their shape, whereas round implants exhibited positional deformation. Softer gel implants showed greater projection increase and MPP shift.

 These findings suggest that when selecting implants, surgeons should consider how different gels behave in upright positioning. This study provides insights to optimize implant selection.
