Modified Millingen Procedure for the Treatment of Trichiasis

はじめに

　睫毛乱生は，睫毛の反りの向きや生える位置が通常より結膜側に寄ることで，角膜刺激症状を呈する疾患の総称である。1960年ごろまではクラミジア感染による慢性結膜炎に続発する病態が主であったが，抗菌薬が発達してまれな疾患になった。しかし，近年は現代ならではの原因（アレルギー性結膜炎，緑内障治療薬の副作用など）で発症していると考えられる高齢患者が増えている。治療法は，概念的には乱生毛の除去か移動であり，臨床的にはいくつかの選択肢があるが，その適応と選択に関するエビデンスは乏しいのが実情で，改善の余地がある。

　The prevalence of trichiasis, which can cause corneal irritation symptoms, is expected to increase with the aging of populations worldwide. Like epiblepharon and palpebral entropion, trichiasis is treatable by surgery and could thus be approached with plastic surgery. Although various surgical methods have been used for the removal of misdirected eyelashes, there have been few studies of the appropriate surgical indications and case selection for trichiasis. This paper describes the authorʼs experience with trichiasis cases treated with a modified Millingen procedure, which has been largely overlooked in Japan.
