Introduction: Postoperative dry eye is a known complication of ptosis surgery. We investigated changes in the Dry Eye-related Quality of Life Score (DEQS) between before and after ptosis surgery.

Patients and Methods: We retrospectively analyzed the cases of 34 patients who underwent levator aponeurosis advancement surgery for the correction of ptosis at our hospital between February and December 2022. The 15 DEQS questions were categorized into four subtypes, and we analyzed the degree scores.

Results: The patientsʼ mean preoperative DEQS score was 30.4 and the median was 28.3. Their mean postoperative DEQS was 10.1 and the median was 7.5. All but two of the 34 patients showed improvement, with mean and median score improvements of 19.4 and 20.0, respectively. Our examination of the preoperative and postoperative degree scores by DEQS subtype revealed that the scores for the items related to symptoms of ocular dryness and foreign-body sensation were unchanged or worsened in about one-half of the patients. The other two subtypes, i.e., eye strain and eye discomfort related to daily life, showed a trend toward improvement in postoperative scores.

Discussion and Conclusions: The DEQS is useful as a dry-eye assessment tool because it is a simple way to assess both quality of life and a wide range of symptoms. However, the DEQS should be used with caution when evaluating dry-eye symptoms that may occur after ptosis surgery, because focusing solely on changes in the DEQS may miss exacerbations of symptoms such as foreign-body sensation and ocular dryness.
