 Tongue cancer is associated with several etiological factors including tobacco smoking, excessive alcohol consumption, genetic factors, and chronic irritation due to a tooth/teeth issue. These factors may also contribute to the development of a second primary carcinoma derived from the skin flap used for tongue reconstruction, although this is rare. A 46-year-old woman underwent a right hemiglossectomy and reconstruction using a rectus abdominis musculocutaneous flap for squamous cell carcinoma of the right side of the tongue. Although she had no recurrent carcinoma, frequent self-biting of the flap was concerning in this patient. At 12 years after the reconstruction, she had developed an ulcer of the reconstructed flap and was diagnosed with a second primary squamous cell carcinoma. The patient underwent tumor resection, and the tooth that had caused skin-flap irritation was extracted the following month. She has had no recurrence as of 1 year postoperatively.
