Treatment of Omphaloceles: The Role of Plastic Surgery

はじめに

　臍帯ヘルニアは，主に小児外科で生後早期に修復術が施行され，形成外科が主体的にかかわる機会は多くないと考えられる。しかし創傷治癒の専門科として，皮膚欠損が大きく標準的な方法では閉創が困難な場合には，修復術前後にコンサルテーションを受けることがある。また形態を再建する専門科として，修復術後の腹壁瘢痕ヘルニアや，臍欠損に対する臍形成術にかかわることがある。本稿では，自施設での経験と文献的考察に基づき，臍帯ヘルニアの治療における形成外科の役割を述べる。

　Omphaloceles are repaired mainly in the early postnatal period by pediatric surgeons, and there are not many opportunities for the involvement of plastic surgery. However, as specialists in wound-healing, plastic surgeons may be consulted when the skin defect is too large to be closed by standard methods. Moreover, in the reconstruction of abdominal wall or the umbilicus, plastic surgeons may be involved as specialists in reconstruction and aesthetics. This article describes the role of plastic surgery in the treatment of omphaloceles based on experiences at our own institution and provides a review of the relevant literature.
