異所性蒙古斑に対する全身麻酔下レーザーの照射面積と診療報酬点数についての検討

The Area of Laser Irradiation and the Number of Reimbursement Points Under General Anesthesia for Ectopic Mongolian Mongolian Spots

はじめに

　異所性蒙古斑は異所性メラノーシスの1つであり，Qスイッチ付きレーザーの照射が有効である 1）。当施設は多くの異所性蒙古斑の症例が集まっており，中には病変が広範囲に及び照射に際して全身麻酔を要する症例も存在する。現行の医科診療報酬点数 2）（表）に沿って算定すると，小範囲で多部位にわたる異所性蒙古斑の症例よりも，1部位で広範囲にわたる症例の方が低い診療報酬点数となることが問題と考えられる（図1）。今回われわれは照射に際して全身麻酔を要した症例について，実際に照射した面積と部位および得られた診療報酬点数について分析し，現状を考察したので報告する。

　Some cases of extensive ectopic Mongolian spots require general anesthesia for treatment. Ectopic Mongolian spots tend to naturally fade over time, but they can persist into adulthood. An analysis of the irradiated area and medical reimbursement points for Q-switched laser therapy for ectopic Mongolian spots conducted at our hospital revealed that approximately half of the 168 patients treated during the period Jan. 2017 through Dec. 2021patients did not receive appropriate compensation for the treatment. A new medical reimbursement point system for Q-switched laser therapy is desired.
