毛根を含めた植皮による眉の再建

Reconstruction of the Eyebrow by Free Grafting of Hair-Bearing Skin

はじめに

　眉は喜怒哀楽の表情表出に重要な役割を担っており，その欠損は周囲の人に奇異な印象を与えるだけでなく，本人にとっても大きな精神的な負担になる。患者と医療者双方が満足できる眉を再建するには，眉毛の密度や太さおよび流れを考慮した自然な眉の形成を心がけることが重要である。

　毛を用いた眉の再建材料としては，眉毛の密度の点から，欠損側の残存眉毛や健側の眉毛もしくは毛髪が選択される。手術方法としては，遊離植皮術，遊離植毛術，局所皮弁術および動脈島状皮弁術などがある。

　本稿では毛根を含む植皮術による眉の再建を取り上げ，手術法や生着率を向上させる工夫および有茎皮弁との比較などについて報告する。

　The eyebrows are essential to facial expression. Many factors must be considered in the satisfactory repair of an eyebrow defect, including density, thickness, and flow, as well as shape of the brow. Free composite graft of hair-bearing skin is one of the available methods for such repairs. The procedure is convenient and usually can be accomplished in one surgery. The postauricular and preauricular scalp area is preferably chosen as the donor site, because of its resemblance to the eyebrow. When eyebrow and eyelid are simultaneously lost, replacement of combined composite scalp and postauricular or preauricular skin graft taken in continuity is an effective procedure for one-stage reconstruction of both.
