OASIS Ⓡ細胞外マトリックスの使用経験—臨床応用における特徴と今後の展望—

Experience with OASIS Extracellular Matrix: Characteristics of Clinical Application and Prospects

はじめに

　組織欠損や難治性潰瘍の治療において，創傷治癒を促進させるために利用する材料として，コラーゲン使用人工皮膚（人工真皮）が知られている。OASIS Ⓡ細胞外マトリックス〔クックメディカルジャパン社，（porcine small intestinal submucosa：以下，SIS）〕は1993年， Badylakによって報告された，ブタ小腸粘膜下組織を脱細胞化した製品である 2）。本邦では2014年に承認されている。先行製品と比べると，生体内の三次元構造を保ったままである，成長因子や構造蛋白などの構成成分が保持されているといった特徴をもつ 3）4）。実際に臨床応用すると，肉芽形成の足場となるばかりでなく，良い上皮化も導くことができるように見え，実際に小さな組織欠損，潰瘍に適用した場合，SISのみで治療を完了できる場合も多く経験した。10例の組織欠損，皮膚潰瘍に対してSISを使用した結果について考察とともに報告する。

　Collagen-based artificial skin （artificial dermis） is used to promote wound healing in the treatment of tissue defects and intractable ulcers. OASIS Ⓡ extracellular matrix （i.e.,, porcine small intestinal submucosa ［SIS］） was first reported in 1993 by Badylak et al., and its use was approved in Japan in 2014. Compared to its predecessors, it is characterized by the retention of the in vivo three-dimensional structure and components such as growth factors and structural proteins. When applied clinically, SIS not only provides a scaffold for granulation; it also gives the impression of good epithelialization and can be applied to small tissue defects and ulcers. The results of 10 cases of tissue defects and skin ulcers treated with SIS are reported and discussed.
