Fat Grafting to the Forehead and Temporal Regions for Facial Rejuvenation

はじめに

　加齢に伴って骨格が変わり，皮膚が余剰となり，さらに脂肪の減少も進んでくる 1）。脂肪注入術は，これらの加齢に伴う肌の張りの減少や輪郭形成の改善に大変役立つ。さらに，脂肪組織に含まれる脂肪幹細胞による新生血管促進効果などを基盤とした組織の肥沃化 2），そして肌質改善の効果も期待できる。このため，エイジング治療としての役割は大きいと考えられる。

　本稿では，前頭部（額）と側頭部（こめかみ）に対する脂肪注入の基礎的テクニックと注入手技のキーポイントについて述べる。

　Aging causes losses of facial soft tissue and bony volume, and it reduces skin quality. One of the best methods for facial rejuvenation is thus to restore the younger contour of the face. Fat grafting is useful for facial rejuvenation because it contributes to contour reconstructions, and the grafting of adipose tissue can improve the skin texture.

　The forehead and temporal hollows are typical areas that can cause a patient to feel old, and fat grafting to these hollows is effective. However, in such cases the surgeon must avoid inflicting a nerve injury such as those involving a temporal branch of a facial nerve and supraorbital nerves.

　I provide the details of several fat-grafting methods that can be used to avoid nerve injury complications, including procedures for fat harvesting, processing, and injecting.
