36Gy/6回/6週の週1回局所膀胱放射線治療で完全寛解が得られた高齢者筋層浸潤性膀胱癌の1例

Complete response to radiotherapy alone with 36 Gy in 6 fractions in an elderly patient with muscle-invasive bladder cancer：a case report

今回我々は，根治的膀胱全摘術を希望せず放射線治療単独の方針となった，90代女性の筋層浸潤性膀胱癌（cT3bN0M0）の1例を経験したので報告する。症状緩和と局所制御を目的として36Gy/6回/6週の局所膀胱放射線治療を提案し，実施した。急性期有害事象はなく，治療終了から3カ月時点で完全寛解が得られた。本症例において36Gy/6回/6週の膀胱放射線治療は安全で有効であった。

We report a case of muscle-invasive bladder cancer（cT3bN0M0）in a woman in her 90s. Because she did not wish to undergo radical cystectomy, she was treated with radiotherapy alone. Local bladder radiotherapy of 36 Gy in 6 fractions over 6 weeks was administered for symptom relief and local control. No acute adverse events were observed, and complete response was achieved 4 months after treatment. This case suggests that bladder radiotherapy of 36 Gy in 6 fractions over 6 weeks may be a safe and effective treatment option for frail elderly patients.
