Pancreatitis and pancreatic cystic lesions

日常診療において，膵炎あるいは膵嚢胞性疾患に遭遇する機会は比較的多い。今回の特集のテーマは「基本とアップデート」であるが，いずれも比較的common diseaseであり，基本的な内容はすでに理解されている読者も多いだろう。本稿では，急性膵炎および膵管内乳頭状粘液腫瘍（intraductal papillary mucinous neoplasm：IPMN）に焦点を当て，「アップデート」された最新のガイドラインを概説するとともに，基本事項をあらためて整理していくこととする。

This review outlines recent updates in clinical guidelines for acute pancreatitis and intraductal papillary mucinous neoplasm（IPMN）, focusing on imaging-related aspects relevant to daily practice. For acute pancreatitis, the revised international guidelines emphasize appropriate indications and timing of CT, limited use of routine follow-up imaging, and complementary roles of MRI and EUS. For IPMN, the updated Kyoto guidelines refine high-risk stigmata and worrisome features and simplify surveillance protocols based on cyst size, while also addressing when surveillance may be discontinued. These updates help optimize imaging strategies and clinical decision-making.
