BCGワクチンの副反応として接種部の対側肘部に生じた皮下結核性肉芽腫の1例

A case of subcutaneous tuberculous granuloma in the elbow opposite to the injection site as a side effect of BCG vaccination

BCGワクチンはウシ型結核菌を231世代継代培養した生ワクチンで，髄膜炎や播種性結核などの重症結核を予防し，結核感染による致死率を64～78％低下させるとされ結核蔓延国において広く用いられている。結核中～低蔓延国である我が国では弱毒化ウシ型結核菌東京No. 172株が使用されており，この株は毒力が弱く副反応も少ないとされている^1）。BCGワクチンの副反応の多くは局所の潰瘍やリンパ節炎であり，まれに皮下肉芽腫，播種性皮膚病変，骨髄炎などが知られている。厚生労働省による2013年の報告では，副反応の発生率は約0.02％とされる^2）。今回我々は，BCG接種後に接種した上腕と対側の肘部に皮下組織結核性肉芽腫を生じた症例を経験したため，文献的考察を加えて報告する。

This is an unusual case of a 1-year-old child with a slowly progressive mass in the right elbow ultimately diagnosed as a subcutaneous tuberculous granuloma following BCG vaccination. Complications following BCG vaccination are limited to infancy and may present with characteristic imaging findings. Although rare, recognition of these complications is crucial to ensure prompt diagnosis and early treatment.
