小児泌尿生殖器には膨大な疾患があるが，本稿では，腎芽腫についてと，超音波エラストグラフィの精巣への応用について取り上げる。腎芽腫については，2022年より日本小児がん研究グループ（Japan Children’s Cancer Group：JCCG）の腎腫瘍委員会がInternational Society of Paediatric Oncology（SIOP）の国際共同研究に参加していることから，そのプロトコルに沿って治療を行う施設が増えている。超音波エラストグラフィは，肝臓で多く用いられているが，非侵襲的な方法で簡便に組織の硬度を評価できることから様々な部位で用いられており，精巣でも近年有用性を示す報告が増えてきている。

The SIOP UMBRELLA protocol for nephroblastoma, unlike the COG approach, is based on administering chemotherapy prior to surgery, making it essential to consider the possibility of tumors other than nephroblastoma. The usefulness of diffusion-weighted imaging（DWI）for differentiating nephroblastoma and predicting its prognosis is highly anticipated. Recently, the utility of ultrasound elastography for various testicular diseases has been reported, and its active use may contribute to better diagnosis and prognosis prediction.
