嗅神経芽細胞腫に対する術後放射線治療を施行した1例

A case of postoperative radiotherapy for olfactory neuroblastoma

嗅神経芽細胞腫は嗅粘膜上皮由来の悪性腫瘍であり，発生頻度は100万人に0.4人とまれな疾患である。術後放射線治療の有用性が示唆されているが，線量・照射野に関して報告は様々である。今回我々は，嗅神経芽細胞腫に対して術後放射線治療を実施した1例を経験したので文献的考察を加えて報告する。

We present a case of a 50s female patient suffering from olfactory neuroblastoma, who had irradiated postoperatively with Volumetric Modulated Arc Therapy（VMAT）, prescribed dose was 66 Gy in 33 fractions. She had three times recurrences in the right parietal lobe and mid skull base as suspected dural extension. Each time with postoperative or stereotactic radiotherapy, we controlled the recurrent tumors. She has been disease-free survival for 18 months after the final re-irradiation. The optimal dose and irradiation field for olfactory neuroblastoma are not known, and we need more cases for getting them. It is necessary to determine the irradiation field in consideration of the risk of dural metastasis and lymph node metastasis.
