今回我々は46,XY性分化疾患の1つであるSwyer症候群に合併した右性腺腫瘤が2カ月間でMRIでの性状変化を伴って縮小し，病理学的に性腺芽腫と診断された症例を経験した。Ⅱ型胚細胞腫瘍の自然退縮は精巣で知られるが，卵巣では知られていない。表現型女性の性分化疾患における異形成性腺での性腺芽腫，Ⅱ型胚細胞腫瘍の自然退縮に関する臨床的，画像的な報告もこれまでになく，MRI所見と病理組織所見とを対比してこれを報告する。

We present a case of spontaneous regression of gonadoblastoma in Swyer syndrome. Magnetic resonance imaging showed significant tumor shrinkage over two months with several changes in appearance：higher intensity on T2-weighted images, less diffusion restriction, and more delayed contrast enhancement. As with type Ⅱ germ cell tumors（GCTs）of the testis, GCTs from dysgenetic gonads may regress spontaneously, even when the internal genitalia show a female pattern.
