血栓閉塞型上行大動脈解離に類似したIgG4関連大動脈周囲炎と後腹膜線維症に伴うiliac compression syndromeを合併した1例

A case of IgG4-related periaortitis/periarteritis like aortic dissection and iliac compression syndrome with retroperitoneal fibrosis

上行大動脈解離は診断が遅れると命にかかわり，不可逆的な合併症をきたしやすい重篤な疾患である。そのため迅速な診断，手術適応の判断，適切な治療を行うことが重要となってくる。ただし，上行大動脈解離と鑑別すべき疾患も中には存在する。今回，突然の下肢腫脹を契機に発見された血栓による偽腔閉塞型上行大動脈解離に類似したIgG4関連大動脈周囲炎と後腹膜線維症に伴うiliac compression syndromeを合併した症例を経験したので，若干の文献的考察を交えて報告する。

Ascending aortic dissection is a serious illness that can be life-threatening and prone to irreversible complications. Therefore, it is important to make a quick diagnosis, determine the indication for surgery, and perform appropriate treatment. However, some diseases should be distinguished from ascending aortic dissection. we reported a case of IgG4-related periaortitis/periarteritis like ascending aortic dissection, with atypical findings and relatively strong FDG uptake.
