画像診断には様々なサインがあり，それぞれがその疾患の特徴的な病態・病理を反映している。そうしたサインを知るとともに疾患の病態・病理を理解することは，画像診断を行ううえで非常に有用である。日常臨床ではしばしば良性病変とがんとの鑑別に苦慮する場面に遭遇するが，特徴的な画像所見を理解することは鑑別の手がかりとなり，不要な侵襲を避けるうえでも非常に重要となる。本稿では肝胆膵領域において，肝細胞癌との鑑別が問題となる限局性結節性過形成，肝細胞腺腫，また膵癌との鑑別が問題となる自己免疫性膵炎に関して，自験例を供覧しつつ特徴的な画像所見を述べる。

There are various signs in diagnostic imaging, each of which reflects the pathology and histological features of diseases. These signs often play an important role in differentiating on imaging from cancer. This section describes the characteristic imaging findings of localized nodular hyperplasia（central scar, spoke-wheel appearance）, hepatocellular adenoma（atoll sign）, and autoimmune pancreatitis（sausage-like appearance, capsule-like rim, speckled enhancement, duct-penetrating sign, icicle sign）, by sharing representative images.
