消化管出血で発見されたキャッスルマン症候群によるアミロイドーシスの1例

A case of amyloidosis with acute intestinal hemorrhage associated Castleman syndrome

消化管に親和性の高いアミロイドは，全身性アミロイドーシスが多く，免疫グロブリンを前駆物質としたamyloid light chain type（AL）型と，炎症，感染時に増加する急性期蛋白serium amyloid A proteinを前駆蛋白としたAA型が多い。今回消化管出血で発見され，臨床的にキャッスルマン症候群に併発したAL型アミロイドーシスの1例を経験した。

We report a rare case of intestinal involvement of AL amyloidosis associated with Castleman syndrome. Abdominal CT images show small intestinal wall thickening with submucosal edema and dilatation, colon wall thickening, hepatomegaly with diffuse poor enhancement of right lobe and abdominal lymphadenopathy. intestine consistent with AL amyloidosis. Intestinal and liver biopsy reveal AL amyloid deposition.
