大腿骨頭軟骨下脆弱性骨折（SIF）は骨粗鬆症を伴った高齢者に発生することが多い脆弱性骨折の一つである．保存的治療で治癒する場合もあるが，急速に骨破壊が進行する症例もある．寛骨臼骨折の保存的治療中にSIFを発症し，人工股関節全置換術（THA）を行った高齢者の1例を経験したので報告する．
