We report surgical techniques and tips for uniportal robot-assisted thoracic surgery (u-RATS) using the da Vinci Xi system for lung cancer and mediastinal tumors. Lung resection is performed in the lateral position, and a 4 cm incision is made at the mid-axillary line in the 6th or 7th intercostal space. Three arms are used, with the dorsal arm being canceled. Insert the camera to the dorsal side, and insert the left and right arms parallel to each other. The camera uses a 0 degree camera. The assistant uses two forceps through the gap between the inserted forceps to expand the surgical field and perform suction. When using a stapler, remove one arm and use the da Vinci stapler. u-RATS using the subxiphoid approach for anterior mediastinal tumors is performed with the patient in the supine position, with the arms of the left and right hands crossed within the subxiphoid wound, changing the allocation of the left and right hands. u-RATS sometimes has operational limitations, but with ingenuity it enables less invasive and more advanced surgery.
