要旨
目的:レスパイトケアを利用する前と利用してからの,母親のきょうだいに対する思いの変化を明らかにする.
方法:近畿3府県において,レスパイトケアを利用している在宅重症心身障害児(者)の母親15名を対象に,きょうだいへの思いに関する半構造化面接を行い,逐語録を作成し質的記述的に分析した.
結果:レスパイトケアを利用する前は【きょうだいに対する自責の念】【子どもに平等に接したい思い】の2カテゴリー,レスパイトケアを利用してからは【きょうだいを喜ばすことができる嬉しさ】【きょうだいのポジティブな気持ちの変化に対する喜び】の2カテゴリーが抽出された.
結論:レスパイトケア利用により,母親はきょうだいを喜ばせられる嬉しさで愛情が満たされ,きょうだいの障害児への愛情を育むという喜びにもつながっていた.レスパイトケアは,母親ときょうだいの関係性の再構築を促進し,情緒的な結びつきを強める上で重要であった.
Objective: The purpose of this study was to clarify the change in mother's thoughts of siblings of children (individuals) with severe motor and intellectual disabilities living at home before, and after beginning the use of respite care.
Methods: The data were obtained from fifteen mothers of children with severe motor and intellectual disabilities in three prefectures of the Kinki area using semi-structured interviews. The interview data was recorded and analyzed qualitatively and inductively.
Results: The qualitative inductive analysis of the information pertaining to emotions before beginning the use of respite care produced two categories. They are [Remorse for the treatment of siblings of disabled children] and [I want to treat my children equally]. The emotions analyzed after beginning the use of respite care produced an additional two categories being, [The happiness felt by bringing siblings joy], and [The joy of seeing the siblings' feelings change positively].
Conclusions: By using respite care, the mothers were able to experience the happiness of making the siblings feel joy. Through this the mothers were able to satisfy their own love, which also had the joy of helping the siblings to grow their love for the disabled children. Respite care was important in facilitating the restructuring of mother-sibling relationships and strengthening emotional ties.
