要旨：てこの原理を応用して回転筒の円周方向へ牽引を行う回転型動的牽引スプリントを開発した．本スプリントの特徴は，MP関節伸展位拘縮に対して関節裂隙を拡大する牽引を行うことである．従来のスプリントの使用では示指MP関節伸展位拘縮が残存したケースに，本スプリントを使用したところ良好な結果がえられた．本スプリントは，関節内骨折などによる関節内拘縮が原因で手指MP関節に伸展位拘縮が生じている場合，関節裂隙を拡大させることに有用なスプリントであると思われる．

　We have developed an A-revolving type of dynamic traction splint that can be pulled to the circumference direction of the rotary pipe. A characteristic of this splint is that traction can be performed to extend joint space for an extension contracture of the metacarpophalangeal joint. When an extension contracture occurs in this joint, the A-revolving splint will effectively widen the joint space.

　We used one with good results on a patient whose index finger extension contracture of the metacarpophalangeal joint did not improve with a conventional splint.
