 Canadian Occupational Performance Measure (COPM) was applied to a Japanese case with chronological left hemiplegia after stroke. The client actively participated in the occupational therapy program when problems were identified by him. COPM, which evaluates clients' perceptions, provided a frame of client centered practice. COPM would be a useful evaluation tool for documentation of occupational therapy outcomes because it shows the changes in clients' perceptions as a score between initial evaluation and reevaluation.
