精神障害者グループホームにおける作業療法士の支援─長く自宅閉居状態にあったケースが活動性を取り戻すに至った経緯

Supporting occupational therapists in group homes for mental disorders: Recovering from prolonged social withdrawal

要旨：本報告の目的は，精神障害者グループホームにおける作業療法士の役割を検討することである．今回，被害的な幻聴や体感幻覚，注察妄想から長く自宅閉居状態にあった統合失調症の男性に対し，グループホームの職員として介入した結果，ケースは活動性を取り戻し，単身アパート生活へ移行することができた．①対象者から語られる希望や実現したい作業の文脈を汲み取り，それを具体化する活動を提案すること，②「具体的な活動」を共に行うことを通じて関係性を構築し，体験の積み重ねを促進すること，③自尊心の回復と，新たな社会技能獲得への動機づけを引き出すことの3点がグループホームにおける作業療法士の役割であることが示唆された．

　The purpose of this study was to consider the role of the occupational therapist in group homes for mental disorders. The subject was a schizophrenic patient suffering from prolonged social withdrawal due to auditory and cenesthesic hallucinations and delusions. Due to occupational therapy support, the patient recovered the activities and was able to live independently. The results suggest three roles of the occupational therapist in the group home: 1) Understand what the clients want to do and propose how to realize it; 2) Build a relationship through performing “concrete activities” and promote accumulation of the client's experience, and 3) Help to recover clients self-esteem and motivate them to acquire new social skills.
