作業選択意思決定支援ソフト（ADOC）を用いた失語症のあるクライエントと作業療法士との意味のある作業の共有

Shared decision-making between clients with aphasia and occupational therapists: Using Aid for Decision-making in Occupation Choice (ADOC)

要旨：我々は，作業に焦点を当てた目標設定における意思決定を共有するためのiPad用アプリケーションである作業選択意思決定支援ソフト（ADOC）を開発した．ADOCでは，日常生活上の作業場面のイラスト94項目の中からクライエントにとって重要な作業をクライエントと作業療法士がそれぞれ選び，協業しながら目標設定を行う．今回，重度失語症のA氏に対してADOCを用いた結果，A氏が意味のある作業に気付き，作業に焦点を当てた実践へと展開するきっかけを作ることができた．この経験から，ADOCは意思疎通が困難なクライエントとの作業に焦点を当てた意思決定の共有を促進する有用なツールであることが示唆された．

　We have developed an iPad application (Aid for Decision-making in Occupation Choice - ADOC) to facilitate shared decision-making in occupation-based goal-setting processes between clients and occupational therapists. In the ADOC, of the 94 illustrations described in daily activities, both clients and occupational therapists chose important activities and set the occupational therapy goal. In this case study, the client with severe aphasia could perceive and engage in a meaningful occupation for himself through ADOC. This suggests that ADOC probably facilitates communication with clients by sharing the　decision-making　in occupation-based goal-setting processes.
