要旨：作業選択意思決定支援ソフト（以下，ADOC）は国際生活機能分類（ICF）の活動・参加レベルの項目に準拠した95項目のイラストを用いて，クライエントと作業療法士が協働的に目標設定を行うiPadアプリケーションである．今回，入院生活により生活全般に主体性を欠く重度の認知症事例に対し作業療法士の役割や作業療法目標の理解を促進する目的でADOCを使用した．結果，事例にとって意味のある作業に焦点を当てた目標設定における意思決定の共有が容易になり，効果的な協働が可能となった．認知機能の低下したクライエントにADOCを用いることでクライエントが積極的に意思決定に参加し，効果的な協働が促進される可能性が示唆された．

　The iPad application, Aid for Decision-making in Occupation Choice (ADOC), was developed to promote shared decision-making during occupation-based goal setting. It involves choosing from 95 illustrations describing daily activities related to the item “activities and participation” in the International Classification of Functioning, Disability, and Health. By using ADOC, clients with severe dementia can participate and share in the decision-making during goal setting, leading to collaboration in achieving meaningful occupation. It suggests that ADOC can help clients with declining cognitive function in sharing the decision-making in the occupation-based goal-setting process.
