仰臥位による後腹膜腔鏡下腎尿管全摘除術

Retroperitoneoscopic nephroureterectomy for transitional cell carcinoma of the upper urinary tract:A new approach in a supine position

　目的：仰臥位で施行する後腹膜腔鏡下腎尿管全摘除術の有用性を検討した．【対象と方法】腎盂癌ならびに尿管癌と診断された5症例を対象とした．手術体位は仰臥位で，患側の腸骨稜の2横指内側に3〜4cmの皮切をおき，後腹膜腔を展開したのちラップディスクミニ®を装着した．あと2本のトロッカーを設置して腎摘除を行った．遊離した腎を最初の皮切部から体外に摘出し，その創から直視下に下部尿管を処理した．【結果】5例中1例は尿管周囲の癒着のため腎茎血管処理以降は開放手術に移行した．手術時間は平均353.4分，出血量は平均367.4gで輸血を必要とした症例はなかった．【結論】本法は症例を選べば今後，標準術式の1つになり得ると考えられた．

Purpose: We describe a new technique of retroperitoneoscopic nephroureterectomy in a supine position for upper urinary tract transitional cell carcinoma. Materials and Methods: Between March and December 2002, we performed 5 retroperitoneoscopic nephroureterectomies in patients with tumors of ureter and renal pelvis. The patients were placed in the supine position during the entire procedure. Extraperitoneal space was created by finger dissection and balloon dilation via a 3-to 4-cm iliac skin incision.
