高度肥満に対するハンドアシスト法を用いた腹腔鏡下胃バイパス術

Hand-assisted Laparoscopic Gastric Bypass for Morbid Obesity

　重症肥満の外科治療にminimally invasive surgeryを目的として腹腔鏡を導入し，ハンドアシスト法を用いた腹腔鏡下胃バイパス術を施行した．まず，上腹部に腹腔鏡用ポート，ハンドポート，操作ポートを配置した．胃バイパス術では，胃を水平に離断することにより口側に約30mlの胃嚢を作製し，胃嚢と挙上空腸を結腸前性，Roux-enY式に自動吻合器を用いて吻合した．器械吻合ではアンビルを経口挿入法により胃嚢に留置した．過去2年間に3例の腹腔鏡下胃バイパス術を施行したが，腹腔鏡の利用により比較的安全で低侵襲下に切離や吻合を行うことができた．

To make surgery safer and shorter we have started to apply the laparoscope for gastric bypass, which is the most reliable operation for morbid obesity. At first, we placed several ports including a hand port. The stomach was transected to create a gastric pouch about 30 ml in volume, 2 cm below the esophago-gastric junction, then the Roux limb of gastric bypass was brought up to the pouch in an antegastric fashion. The gastrojejunostomy was constructed using a circular stapler with our tranceoral method for anvil placement in the pouch.
